A Note for New Clients:

You are welcome to print your intake, fill it out at your leisure, and bring it with you to your first appointment.  

If you don't have a printer, no problem. I'll always have a copy available at the office. In that case, I ask that you arrive at least 10-15 minutes early to complete it before your session.

Both of the forms below are required in order to bill insurance for massage therapy.

Before your appointment, I’ll need:

-- Both completed insurance forms 

---A referral from your healthcare provider 

-- A photo of your driver’s license/ID and insurance card emailed to nikky@wildquartzwellness.com at least 3 days prior to your appointment 

Please note: Insurance cannot be billed until I’m able to confirm your coverage for services at my office.

Chiropractors, naturopaths, and acupuncturists can often help with referrals.
