Intake Forms

A Note to New Clients:


You are welcome to print your intake, fill it out at your leisure, and bring it with you to your first appointment.  


Don't have a printer? We'll always have the intake form available here and we ask that you arrive at least 10-15 minutes early to fill out before your appointment.

INSURANCE FORMS

Both of these forms are required for billing insurance. 


We'll need a referral from your healthcare provider in order to bill your insurance for massage. 

Chiropractors, Naturopaths, Acupuncturists can also help with this.


Please also make sure to email nikky@wildquartzwellness.com with a photo of your Drivers License/ID and Insurance Card so we can check your benefits at our office.

We are open by appointment only and do not take walk-ins.


If you're looking for a same day appointment, please check our schedule online to make an appointment.
