A Note to New Clients:

You are welcome to print your intake, fill it out at your leisure, and bring it with you to your first appointment.  

Don't have a printer? I'll always have the intake form available here and I ask that you arrive at least 10-15 minutes early to fill out before your appointment.

Both of these forms are required for billing insurance. 

I'll need a referral from your healthcare provider in order to bill your insurance for massage. 

Chiropractors, Naturopaths, Acupuncturists can also help with this.

Please make sure to email nikky@wildquartzwellness.com with a photo of your Drivers License/ID and Insurance Card at least 3 days prior to your appointment. Insurance cannot be billed before I can confirm that you are covered at my office.
